Office Staff Hours:

The office staff is available to receive phone calls beginning at 9 am and until 15 minutes prior to closing.  Staff is not available during lunch and dinner hours.

Insurance

We are participating providers with several insurance companies:

Horizon BC/BS:  Traditional Plan and Dental Option Plan

Delta Dental:  Premier Plan, Premier+, and Advantage Plan

Cigna:  PPO Plan

Please note that other plans such as MetLife PPO, Aetna PPO, United Healthcare and many others can be used in our office at the out of network rate.  For most PPO plans, this is the same rate as the in network rate.  Please check with your carrier, or contact us if you have any questions.
